@article{oai:meio-u.repo.nii.ac.jp:00001387, author = {玉井, なおみ and 木村, 安貴 and 大城, 凌子}, issue = {23}, month = {2020-12-03}, note = {【目的】本研究は、終末期看護教育の受講前後で看護学生の終末期ケアに対する意識の変化を明らかにし、大学教育における終末期看護教育の有用性および教育方法の示唆を得ることを目的とする。【方法】終末期看護教育プログラム(ELNEC-J)の視点を取り入れた終末期看護教育(選択科目)の受講生18名に対し、終末期ケアに対する意識の変化を医療者のターミナルケア態度尺度日本語版(FATCOD-From B-J)と自由記述で調査した。FATCOD-From B-Jの総得点と下位尺度は、受講前後の中央値[4分位範囲]を用い、統計ソフトSPSS Statistics ver.25を用いてWilcoxonの符号付き順位検定を行い、有意確率はp<.05とした。自由記述は内容分析を行った。【結果】17名から回答を得た。総得点は受講前112[106-115]から受講後127[119-130]と肯定的に変化した(p<.001)。下位尺度の受講前後の変化は「死にゆく患者へのケアの前向きさ」54[51-58]から67[62-70](p<.000)、「患者・家族を中心とするケアの認識」54[51-55]から56[53-60](p<.021)、「死の考え方」4[4- 4]から4[4-5](p<.065)であった。自由記述では終末期ケアについて暗く悲しいイメージから暖かく前向きで明るいイメージに意識が変化した。【結論】ELNEC-Jの視点を取り入れた終末期看護教育は,看護学生の終末期看護の肯定的イメージへ意識が変化しており、大学教育における終末期看護教育の有用性が示唆された。, Purpose: The purpose of the present study is to clarify changes in nursing student’s attitudes of terminal care from before to after attending a terminal care class, in the aim of obtaining suggestions for the effectiveness of terminal care classes and education methods in university education. Methods: The subject sample included 18 students who took a course in end-of-life nursing care that adopted the End-of-Life Nursing Education Consortium - Japan (ELNEC-J). Investigations included the Japanese version of the Frommelt Attitudes Towards Care of Dying scale for medical professionals conducted a total of three times, i.e. before and after the course, then two weeks later, as well as using free descriptions. For total scores and subscales, pre- and post-course scores were compared by Wilcoxon signed rank test using the statistical software SPSS Statistics ver.25 with median values [interquartile range], and a p value of < 0.05 was considered statistically significant. Content analysis was performed for qualitative data. Results: Before and after the course, responses were obtained from 17 students. Total scores significantly increased from 112 [106-115] pre-lecture to 127 [119-130] post- lecture(p<0.001). Subscale scores included 54 [51-58] pre- lecture to 67 [62-70] post- lecture(p<0.000) for “positive attitude towards care for dying patients,” 54 [51-55] pre-course to 56[53-60] post- lecture for “perception of patient- and family-centered care (p<0.021), ” and 4 [4-4] pre- lecture to 4[ 4-5] post- lecture for “views on death ( p < 0.065).” The free descriptions revealed a change from a dark, sad image of end-of-life care to a warm, positive, and bright image. Conclusion: End-of-life nursing care education adopting the perspective of ELNEC-J improved students “positive attitude towards care for dying patients,” and helped change to a positive image of terminal care.
