| 内容記述 | 本研究は，在宅がん患者への教育的介入に関する理論的枠組み構築の示唆を得るため，在宅がん患者の痛みの認識と体験の様相を明らかにすることを目的とした． 国外論文はPubMed，CINAHLから“Cancer Pain”&“Patient OR Family OR Caregiver”&“Perception OR Experience OR Interpret”のキーワードで検索した．国内論文は医中誌Webを用いて，「在宅医療or在宅看護」「がん患者or家族」「がん性疼痛」「認識or体験」のキーワードで検索し，国内外で19件の論文を対象とした．在宅がん患者の痛みの体験には，【制御困難な耐え難い疼痛体験】【痛みと生活の折り合いをつけた対処行動】など６つのカテゴリーが抽出された．痛みの認識には，【過去の痛みと向き合った体験から得られる自己効力感】【痛みの増悪に伴う自律の喪失】【痛みと折り合いをつける肯定的な人生の再構築】など６つのカテゴリーが抽出された． 在宅がん患者の痛みの体験の様相は，複数の要因と多様な変化で制御困難ながん性疼痛特有の疼痛体験に対し，生活と折り合いをつけた対処行動を行っていた．痛みの認識の様相は，過去の痛みへの対処行動から得られた自己効力感が分岐点となり，人生の喪失というネガティブな認識と人生の再構築というポジティブな認識が生じていた． |

| 内容記述 | The purpose of this study was to identify aspects of homecare cancer patients' perceptions and experiences of pain in order to suggest the construction of a theoretical framework for educational interventions for homecare cancer patients. The international articles were from PubMed, CINAHL, “Cancer Pain” & “Patient OR Family OR Caregiver” & “ Perception OR Experience OR Interpret”. Domestic articles were searched using the Ichushi Web with the keywords “home health care OR home nursing,” “cancer patient OR family,”“cancer pain,” and “perception OR experience,” and 19 domestic and foreign articles were included. Six categories were extracted for the experience of pain with homecare cancer patients, including［experience of unbearable pain that is difficult to control］ and ［coping behaviors to come to terms with pain and life］. Six categories of pain perception were extracted, including ［self-efficacy from the experience of dealing with past pain］, ［loss of autonomy associated with worsening pain］, and ［positive life restructuring to come to terms with pain］. Aspects of the pain experiences of the homecare cancer patients were coping behaviors that reconciled their lives with the unique experience of cancer pain, which was difficult to control due to multiple factors and various changes. Aspects of pain perception were based on the sense of selfefficacy gained from past pain coping behaviors, which became a turning point, resulting in negative perceptions of loss of life and positive perceptions of rebuilding life. |
