| 内容記述 | 目的：「 どうしても（小遣い銭を）一万円にしてためしてみたい」という長期入院精神障害の意向を尊重した実践内容を，受け持ち看護師の視点から解明することである． 方法： 受け持ち看護師が実践を振り返った面接内容と，看護記録等を，受け持ちから４年間の時系列にそって一覧表に整理し，大小の見出しをつけた． 結果： 実践内容は次のように構造化された．「多職種チームの支援体制を一人体制で引き継ぐ」という流れのなかで，「金銭に関する主体性を本人に返還すること」，このことを通して「患者の主体的な他人への配慮を支持すること」であった． 結論： 長期入院精神障害者の主体性返還に関して，次の４つのことが明らかになった．①受け持ち看護師の一人体制が，患者の主体性を返還する契機となった．②受け持ち看護師は，本人の要望にそうように，それを支持した．そして，③患者が一万円を自由に使う権利を保障した．しかしながら，④一人体制の受け持ち看護師の実践知を病棟全体に波及させることは課題として残った． |

| 内容記述 | Aim: To clarify the content of the practice of respecting the wishes of long-term inpatients with mental disorders who “really want to do it for 10,000 yen （pocket money）”, from the perspective of the nurse in charge. METHODS: Interviews and nursing records in which the nurse in charge reflected on her practice were organised into a chronological list covering the four years since she took charge, with major and minor headings. RESULTS: The practice was organised as follows. ‘taking over the support system of a multidisciplinary team by only one person’; through this, ‘restoring autonomy regarding finances to the person’; through this, ‘supporting the patient’s proactive care for others’. CONCLUSION: The following four points were identified with regard to restoring the autonomy of long-term inpatients with mental disorders. （i） The one receiving nurse system was an opportunity to restore the patient’s autonomy. （ii） The receiving nurse supported the patient’s demands. （iii） The right to spend 10,000 yen freely was guaranteed. However, （iv） disseminating the practical knowledge of the one-receiving nurse system throughout the ward remained a challenge. |
