| 内容記述 | 【目的】 無為・自閉の慢性期統合失調症患者に対する看護実践についての国内文献を検討し，看護実践内容を明らかにし，今後取り組むべき課題を検討する． 【方法】 文献検索には，国外論文はCINAHL，国内論文は医学中央雑誌を用い，キーワードとして「統合失調症」「無為」「自閉」を検索語とし，発行年は指定せず，原著論文を検索した結果18文献を分析対象とした．看護実践内容は質的記述的に分析した． 【 結果】 対象となった18文献は，1998年から2021年に発行されていた．患者と研究介入に至るまでの看護実践では，患者の反応が無くても【看護師の存在を示す】，タイミングを見計らい【会話の糸口を探る】，タッチングを行い【五感に働きかけ反応を確かめる】ことが行われていた．これにより,患者の【関心を見つけ関わる機会を掴む】ことが可能なり，患者の【外に向かう変化に伴う不安を緩和する】看護実践が行われていた. 【結論】 無為・自閉の患者への看護実践では，集団作業療法への参加や日常生活行動の拡大に繋がるまでのプロセスにおいて，患者と看護師の相互交流の影響が大きいことが明らかになった.看護師は言語的コミュニケーションだけでなく，タッチングによる非言語的コミュニケーションを活用しており，今後はタッチングによる看護実践が，患者－看護師関係構築に有効な方法となるのか検討したい． |

| 内容記述 | Objectives: The purpose of this study is to review studies on nursing practices for chronic schizophrenic patients with abulia and autism, to clarify nursing practices, and to obtain findings and suggestions for future research. Methods: Studies abroad were collected from CINAHL and studies in Japan from Ichushi. Retrieval terms were ‘schizophrenia’ AND searching ‘abulia’ OR ‘autism.’ We analyzed eighteen applicable literatures. Surveying these studies, it was suggested that nurses had extreme difficulties in approaching patients in the stage before nursing practical care, and then, fully-worked ideas and plans were required to be performed. Obtained data were analyzed in a qualitative and descriptive approach. Results: The 18 references covered were published between 1998 and 2021. Extracted categories, which were related to prepared caring before practical care was able to be provided, were as followed: nurses adopted continuous attitudes of “demonstrating the presence of nurses” even without reactions from patients, “probed for an opportunity to start a conversation” with good timing, and “confirmed patients’ reactions when nurses worked on the five senses” through the use of touch. These involvements enabled nurses to “be aware of patients’ interests and seize opportunities to interact patients.” Conclusions: Nonverbal nursing care, such as touch, was adopted as well as verbal communication in the step for the creation of contact with patients. We will conduct further studies to examine practical care involving touch as an effective method to establish a relationship between patients and nurses. |
