| 内容記述 | 研究目的は，へき地のA村で乳幼児をもつ母親が妊娠・出産・育児において，どのような思いで出産（出産場所・分娩方法）・育児（心配事や疑問の解決）に関する意思決定をしているのかを明らかにすることである。A村在住の母親５名に半構造化面接を行い，質的統合法（KJ法）に則って質的帰納的に分析した。母親たちは【都市部に比べた住みやすさ】を感じ住環境に満足していたが，北部の妊娠・出産・育児の現状に対し【足りていない専門家からの支援】を感じていた。それゆえに母親たちは専門家から十分な支援が得られず，【不充分なヘルスリテラシー】の結果として【うまくいかなかった妊娠・出産・育児体験】に繋がっていた。【足りていない専門家からの支援】ゆえに【周産母子医療の都市部との格差】を感じた母親は，【希望に沿う中南部の医療施設の利用】という選択に至っていた。また１人目の【うまくいかなかった妊娠・出産・育児体験】が次子の妊娠・出産・育児の際に【対希望に沿う中南部の医療施設の利用】という意思決定に繋がっていた。しかしながら，母親たちは今後もＡ村に住み続けたいと思っており，だからこそ村内で保健師以外にも【気軽に相談できる身近な専門職の存在】を求めていた。A村の住環境は，母親にとって満足できるものであるが，「切れ目ない妊産婦・乳幼児への保健対策」として妊産婦・新生児が専門性の高い産後ケアが受けられるようにしていくことが望まれる。 | |||||

| 内容記述 | The objective of this study was to clarify how mothers feel or choices made concerning pregnancy, delivery and child-rearing of mothers living in a village at rural area. The research participants were five mothers with infants living in the village, and a semi-structured interview was employed. Data were analyzed with inductive and qualitative approach using the KJ method. Research data showed that mothers felt “rural areas were easier to live in than urban areas” with satisfaction in their living environments. Contrarily, mothers felt “resources were insufficient” in the northern area regarding the actual conditions of pregnancy, delivery or child-rearing. Therefore, sufficient support of experts was not provided. The “inadequate health literacy” led to“ unsuccessful or bad experiences during pregnancy, delivery or child-rearing.” Mothers who felt “there was a disparity in perinatal services between rural and urban areas” because of “insufficient resources,” decided to choose “utilization of medical institutions in urban areas to meet their requirements.” In addition, mothers who had“ unsuccessful or bad experiences during pregnancy, delivery or child-rearing” in having their first child, made a decision for the second child of “utilization of medical institutions in urban areas to meet their requirements.” However, mothers thought that they would like to continue living in the village. That was especially why they hoped that “experts whom they could consult with would be close in the village” other than public health nurses. It is desirable that child-rearing support in rural areas require examinations with academic-government cooperation. | |||||
